Do you accept insurance?

We accept Blue Cross Blue Shield PPO (BC/BS IL) and Blue Choice PPO insurance plans. We will submit your claims to insurance on your behalf.

If you are in network with a different insurance provider, we will gladly provide you with an invoice so that you can submit to your insurance company to utilize your out-of-network benefits. Please call your insurance company to determine your out-of-network benefits in advance.
